Nursing News: Nurse-to-Patient Staffing Ratios Affect on Patient Care

Hospitals and nurses in Massachusetts, one of the 34 unregulated states, are arguing over mandatory nurse-to-patient staffing ratios. Some nurses claimed to work 16 hour shifts on mandatory overtime and feel that risks patient safety.

Hospitals, not wanting to confine nursing care to a “one-size-fits-all” ratio, want collaboration between hospitals, physicians and nurses to create best practices based on quality, not volume.

Meanwhile, 78% of nurses are spending one-quarter of their 12-hour shifts on non-patient care, such as completing checklists and paperwork, according to a survey conducted by Atlanta-based Jackson Healthcare.

According to the nurse respondents, reduced nurse-to-patient ratios and increased ancillary support staff could help shrink the amount of time they spend on indirect patient care.

Nursing News: Life Balance Alert! Maryland Study Finds 55% of Female Nurses Obese

We’re in a life balance crisis within the nursing profession. Researchers at the University of Maryland’s School of Nursing found 55% of the 2,103 female nurses they surveyed were obese. The nurses cited job stress and the effect on sleep of long irregular work hours as the cause. Long work hours and shift work adversely affect quantity and quality of sleep, which often interferes with adherence to healthy behavior and increases obesity.

nurse life balance The study, which measured obesity using estimates of body mass index, found that nursing schedules affected not only the health of the nurses but the quality of patient care.

To combat the high obesity rate among nurses, Kihye Han, the author of the study, proposed more education on good sleep habits, and better strategies for adapting work schedules. She also called for napping at work to curb sleep deprivation, reduce fatigue and increase energy.

The 2004 National Sample Survey of Registered Nurses found that more than 40 percent of nurses who left nursing said they did so because of irregular and long hours, indicating that better scheduling could help nurse retention.  Han also proposed making healthy food more available and allowing enough time to consume it.

The study, published in The Journal of Nursing Administration, is part of a series of investigations that show long work hours and shift-work affects the health of nurses that can also lead to poor outcomes for patients.

Nursing News: Mandating Caps on Overtime Can Drastically Improve Nurse Retention and Nurses’ Life Balance

“Too much to do, too little time” is a phrase that most nurses know well. Nurse retention and life balance are highly affected by being over-worked.Nurse retention

State-mandated caps on nursing overtime hours improve patient safety and care quality, concluded a new study, reported Renal Business Today. The regulations prevented fatigue from long shifts and the associated errors that endanger the safety of patients and nurses.

In the 16 states regulating overtime as of 2010, newly registered nurses were 59% less likely to work mandatory overtime than those in unregulated states and worked an average of 50 fewer minutes per week, according to the RN Work Project study.

The study also found that limiting mandatory overtime enhanced nurse retention, enticing nurses to remain in their jobs and in the profession longer.

Nursing News: Parish Nursing Promotes Wellness and Community

Faith community nursing, or parish nursing, is a specialty of nursing focusing on helping patients achieve health, healing and wholeness through health promotion and disease prevention. Parish nurses serve their congregations and communities by providing referrals to health resources; doing home visits; leading classes on topics such as nutrition, fitness or bereavement, or just providing a listening ear. Some provide blood pressure checks after church services, and write articles on current health topics for newsletters and bulletins.Parish Nursing

Long before there were retirement homes, community clinics and private hospitals, religious organizations took care of the ill and elderly in their communities. But it wasn’t until the 1980s when the model for parish nursing was developed by a Lutheran chaplain named Granger Westberg. He believed health care transcends physical care, because true healing involves the whole person, mind body and spirit, in the context of their community. He founded the International Parish Nurse Resource Center in 1986. Since then, faith community nursing has become an international movement.

Having a nurse on the staff of a congregation provides a unique forum for health promotion and disease prevention. Members of a church can discuss health concerns with a nurse before it becomes a chronic or serious condition.

Nursing News: 36-Year Women’s Health Study Focuses on Nurses

For more than 35 years, researchers at Harvard School of Public Health have been regularly surveying hundreds of thousands of nurses throughout the United States for research that fundamentally changed our nation’s understanding of women’s health. Now they are looking to study the next generation of nurses, enrolling another 100,000 women, ages 20 and older.Nursing Research from Harvard

The Nurses’ Health Study began in 1976, enrolling about 120,000 nurses ages 30 and up with the goal of studying breast cancer and the long-term effects of taking oral contraceptives. In 1989, another 116,000 women began filling out regular surveys about their lifestyles and medical histories.

The researchers chose nurses for two reasons. They knew the women would be committed to the project and, for the most part, they could trust the nurses’ assessments of their own medical histories.

The results of the studies have been far-reaching, leading to a better understanding of cardiovascular risks in women, risk factors related to birth control, and the dangers of diets high in trans fat. As the women in the first round of the study grow older, researchers are using their data to look at factors contributing to Alzheimer’s disease, other age-related illnesses, and healthy aging.

The goal with the latest recruitment is to attract young women….yet another way nurses serve.

Nursing News: New Study Published about Nurse Retention

The American Journal of Nursing published findings from the RN Work Project, a ten-year longitudinal study of newly licensed registered nurses that began in 2006.

The RN Work Project is designed to learn more about nurses’ career patterns, including turnover and nurse retention. The results of this study were drawn from two surveys of new RNs in 15 states, one conducted in 2006 prior to the recession and the second conducted in 2009 during the recession.

The two groups of nurses were demographically similar, but the second group reported significantly better health status (23 percent rated their health as excellent compared with 19 percent of the first group) and fewer needle stick injuries, sprains, and strains.

The 2009 group also reported working an average of 52 hours less a year, reported better nurse-physician relationships, and perceived the work environment as significantly better. While the RNs surveyed in 2009 reported a higher level of intent to stay in their current jobs, they were also more likely to be searching for a new job than the RNs surveyed in 2006. They also perceived fewer job opportunities than the 2006 nurses.

The researchers note that the nursing shortage, which caused great concern for many years, has lessened in part because the recession has led many older RNs to delay retirement or to return to nursing. As the economy improves, many of those nurses will retire creating greater demand for new graduates.

As the recession eases and the job market opens up again, it’s likely that nurses who have been delaying changing jobs will begin looking for new positions, which could dramatically increase staff turnover. Healthcare organizations should take this opportunity to continue to implement programs that will increase retention.

Healthcare News: Outside Industries Training Workers to Become Nursing Professionals

Former warehouse, factory and autoworkers are still lifting, but trading in their coveralls and job uncertainty for nurses’ scrubs and becoming nursing professionals. Hard figures are elusive, but the Michigan Department of Energy, Labor and Economic Growth estimates a shortage of 18,000 nurses in the state by 2015, and their labor force is adapting.

Oakland University in nearby Rochester, Mich., has established a program specifically to retrain autoworkers in nursing, approximately 50 a year since 2009. The College of Nursing at Wayne State University in Detroit is enrolling a wide range of people, including former manufacturing workers, who are switching to health careers.
